Familial clustering. There appears to be a familial clustering of IgA nephropathy which shows strong family predisposition in about 10% of cases, with this being more common in some specific regions or in families from those regions. In the U.S., these include Kentucky and parts of the Southeast. In other parts of the world, familial clustering of IgAN seems to be more common in Southern France and Italy.

Ethnicity. In addition to the regional-based familial clustering, IgAN is known to be more common in some specific ethnic groups (although it does not necessarily run in families). IgAN is most common in people of Asian ethnicity (oriental), followed by Hispanics and then Caucasians (seems to be relatively more common among the French). It is least common in African Americans. IgAN is also more common in American Indians than it is in whites.

Not familial. Greater than 90% of IgA nephropathy is not familial (in other words, there are no other family members with the disease).

Pediatric IgA Nephropathy Symptoms



Most of the symptoms of Pediatric IgA Nephropathy, or Berger's disease seen in children are similar to those seen in adults, but they are less common or appear in different stages of the disease process.

Hematuria


Gross hematuria can be seen when the patient's urine is smoky or cloudy. The urine is darker than normal because of the red blood cells in the urine.
A strong smell might accompany the dark urine. The hematuria is not constant and occurs only during an active episode of the disease.

Abdominal Pain

Children with Berger's disease often experience bouts of excruciating abdominal pain that can mimic appendicitis. The pain can be in the middle of the abdomen or localize to the right lower quadrant.

Proteinuria

Proteinuria is the presence of large amounts of protein in the urine. The proteinuria is caused by destruction of a part of the kidney.
The kidney usually filters out protein from the urine, but IgA Nephropathy damages the kidney and prevents it from performing this function. Proteinuria sometimes can be evident when a patient notices that his urine is more foamy than normal, due to the presence of protein in the urine.

Focal Segmental Glomerulosclerosis and Gluten Free Diet


Like celiac disease, FSGS is an inflammatory disease, enflaming and scarring the glomeruli, or filtering system, of the kidneys. It was also that summer that I discovered that eliminating gluten from my diet put my disease into spontaneous, full remission, an extremely rare occurrence.




I want to share my experience with others so that others can be aware that eating gluten free does not only benefit those with celiac disease.
How did I get so lucky to go from health to serious illness back to health in a short period of time?

My health crashed & burned in the summer of 2009. I didn’t see it coming. I had seemingly sudden and unexplained weight gain caused by edema, hypertension, and high cholesterol. All signs pointed to a kidney problem, so I immediately put myself on a renal diet that happened to be nearly gluten free. A renal biopsy confirmed the diagnosis of FSGS with nephrotic syndrome.

Most people with the disease eventually progressed to End Stage Renal Disease, requiring dialysis and transplant. Stubbornly I refused to accept this was my future and refused to accept that I couldn’t figure out the cause and a solution.

It was during that time of diagnosis, doing research, checking out theories, going for follow-up tests, that my friend found stories of others with kidney disease who had gone into, and stayed in, remission merely by going on a gluten free diet.

Within five weeks, my tests showed that I was in full, spontaneous remission. It’s important to note that I had already been eating mostly gluten free for three months prior to this full commitment and was already showing vast improvement. It’s important to have a long view when giving this diet a try. My nephrologist called my improvement “remarkable” and “fabulous”. We checked again a month later, and I was even more into the healthy range. Six months later at a follow up visit, my numbers showed healthier than many healthy people without kidney disease. My nephrologist envies my blood pressure and cholesterol. Today I am still in full remission, on no medication, and still gluten free.

While a gluten free diet may help virtually all with celiac disease, it may help only some with chronic kidney disease and other medical conditions. It is not widely known, even by nephrologists, and it is not widely studied. Through word-of-mouth, I have been encouraging others with kidney disease to try this very safe diet, and I am aware of two children and two adults with kidney disease who have gone into remission as a result. Others did not see any improvement or saw unexpected improvements such as weight loss, arthritic pain gone, and insomnia cured.

IgA Nephropathy and Pregnance: Can I Have A Baby With IgA Kidney Disease

Factors that decides whether you can get pregnance or not


Many women with IgA Nephropathy have conceived successfully, however, your nephrologists’ recommendation or advice will depend on your individual circumstances. Factors that may be considered are degree of existing renal failure (that is, what your kidney function is, and if it's less than about 70%), the amount of proteinuria you have, whether you have high blood pressure or not, and also some of your biopsy findings (any sign of significant tubulointerstitial involvement). On the other hand, pregnancy should not be automatically excluded on the basis of having IgA Nephropathy. A recent study involving 60 pregnant women with IgA Nephropathy, the conclusion was that "in pregnancy complicated with IgA Nephropathy, if the renal function before pregnancy was satisfactory, an uneventful course of pregnancy could be expected by careful control of the patient, although the incidence of EPH-gestosis was high". EPH-gestosis means essentially the same thing as pre-eclampsia. A satisfactory renal function in this case would mean a serum creatinine somewhere under 200 umol/L (equivalent to 0.20 mmol/L in some countries, or approx. 2.3 mg/dl in the U.S.), and well-controlled hypertension (on drugs that will not harm the fetus).

So, when it comes to pregnancy, no matter what you read here or anywhere else, the bottom line has to be consulting with your nephrologist, and if necessary, whatever other medical specialists are appropriate in your case.

Is IgA Nephropathy Difficult to Cure

(6) arteriosclerosis score. Results for interobserver reproducibility of individual pathological features are likely applicable to other glomerulonephritides, but it is not known if the correlations between variables depend on the specific type of glomerular pathobiology. 


Variables identified in this study withstood rigorous pathology review and statistical testing and we recommend that they become a necessary part of pathology reports for IgA nephropathy. Our methodology, translating a strong evidence-based dataset into a working format, is a model for developing classifications of other types of renal disease.
